This form is used to request an online course evaluation. Typically, this form is submitted by department-wide personnel, such as academic Department Heads or Administrative Assistants. Online course evaluations are completed by students enrolled in academic courses within a specific department. The requested course(s) will be available at http://learn.nmsu.edu.
