Zoom Telehealth Account Activation or Deactivation Request

NMSU provides a HIPAA compliant Zoom telehealth account by departmental request only. This account can be used to support the delivery of NMSU services, student learning activities such as clinical hours, or research. For security purposes a deactivation request should be submitted once an account is no longer needed.

    Enter only the NMSU usernames (do not include @nmsu.edu) for each account holder or upload a spreadsheet below.
    If you are requesting both activation and deactivation please upload a workbook or two separate spreadsheet files, one for activation and one for deactivation.
